Excess Ballot Reporting Form

The form below is for reporting excess ballots. The Secretary of State has sent out ballots to the address of every registered voter in Vermont. While some voters have moved or passed away, those changes do not always get accurately reported to the voter checklist, which causes ballots to be sent out in error. If you have received a ballot at your address which you believe should not have been sent, please give us the information requested below. Include your contact info so that we can follow up with any questions.

Our town clerks are very busy preparing for a smooth Election Day, but after November 8th, we will refer those incidents reported here to the appropriate town clerk or the Board of Civil Authority in order to challenge the names to ensure that if they are on the check list in error, they can be removed using the process outlined by state law.

The first section of the form is asking for information about the ballot that was sent in error, the second portion is asking for your information, as the person reporting the error.
